The Service Coordinator performs a wide range of tasks to advocate for, and assist residents living in HUD 202 Elderly Housing managed by EveryAge. The Service Coordinator coordinates efforts of outreach, resource coordination, problem solving, intervention, advocacy, community building, and increasing visibility of the services within the broader community.

1. Provides general case management and referral services to all residents needing such assistance including: evaluation of health, psychological and social needs for all residents.

2. Established linkages with all agencies and service providers in the broader community; shops around to determine/develop the best "deals" in service pricing to assure individualized, flexible and creative services for involved residents.

3. Sets up director of providers for use by facility staff and residents.

4. Refers and links the residents of the community to service providers in broader community; including case management, personal assistance, meals on wheels, transportation, counseling, occasional visiting nurse, preventive health screens/wellness and legal advocacy.

5. Educates residents on service availability, application procedures, clients rights, etc.. providing advocacy as appropriate.

6. Develops case plans in coordination with assessment services in the community.

7. Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual. Manages provision of supportive services where appropriate.

8. Establishes volunteer support programs with service organizations in the broader community.

9. Helps residents build informal support networks with other residents, family and friends.

10. Provides training to residents in the obligations of tenancy or coordinates such training.

11. Educates other staff on issues related to aging in place and service coordination, as a means of helping them better work with and assist residents.

Job Type: Part-time

Pay: $19.00 - $20.00 per hour

Expected hours: 20 – 24 per week

Schedule:

  • Monday to Friday

Work Location: Multiple locations
